Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-37-4    Basic - Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom.
36-32-5    Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed water stained ceiling tiles and ceiling tiles missing in dining room and kitchen area.
24-07-4    Basic - Cleaned and sanitized equipment or utensils not properly stored. Observed pots and pans stored in mop sink room with mop bucket/mop and equipment.
21-05-4    Basic - Cloth used as a food-contact surface. Observed cloth used as food contact surface by steam table and throughout kitchen area. **Corrected On-Site**
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed food debris on floor throughout kitchen area especially in corners.
36-14-4    Basic - Grease accumulated on kitchen floor and/or under cooking equipment . Observed throughout kitchen area.
36-24-5    Basic - Hole in or other damage to wall. Observed by handwash sink next to walk-in cooler.
14-69-4    Basic - Ice buildup in reach-in freezer. Observed Ice buildup in interior of IDYLIS Reachin freezer.
10-07-4    Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed container with utensils in standing water by steam table. Operator removed container with utensils. **Corrected On-Site**
22-19-4    Basic - Interior of microwave soiled with encrusted food debris.
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. None observed at handwash sink by servers ' station.
33-38-4    Basic - No waste receptacle installed at handwash sink provided with disposable towels. None observed at handwash sinks in kitchen and dishwashing area.
14-40-4    Basic - Nonfood-contact equipment in poor repair. Observed a True reachin cooler front in disrepair.
36-26-4    Basic - Wall soiled with accumulated black debris in dishwashing area.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed on tables throughout kitchen area.
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. Observed 11 dented can of Kings Bamboo Shoots, 1 dented can of Cream Style Corn, 3 dented cans of Water Chestnuts, 2 dented cans of Tomato Ketchup.
12A-20-4    High Priority - Employee washed hands with no soap. Observed employee washed hands on 3 compartments sink with no soap. Have employee wash hands in handwash sink with soap and hot water.
50-08-7    High Priority - Establishment operating without a license from the Division of Hotels and Restaurants. Operator must obtain a public food service license from the Division of Hotels and Restaurants within 30 days. Submit a license application and payment to DBPR/Bureau of Central Intake Unit, 2601 Blair Stone Rd., Tallahassee, FL 32399-0783. 03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Observed fresh garlic and oil stored at 87 degrees F and cut bak Choy stored at 71 degrees F for 3 hours per operator by cookline. Advised operator to place items in cooler to cool.
08A-05-4    High Priority - Raw animal food stored over ready-to-eat food. Observed raw chicken stored above cooked chicken and raw salmon in Bison reachin cooler, raw chicken above wonton in walk-in freezer. Operator repositioned items - raw chicken to bottom. **Corrected On-Site**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed 4 moist rodent droppings onto of hot water heater next to drain board in dishwashing area and 1 moist on floor under drainboard in dishwashing area.
01B-03-4    High Priority - Stop Sale issued due to adulteration of food product. Observed mold like substance dripping directly onto more than 100 lbs of drink ice in ice machine.
41-25-4    High Priority - Tracking powder pesticide used inside establishment. Observed throughout kitchen area.
29-42-4    High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.
22-20-4    Intermediate - Accumulation of black/green mold-like substance in the interior of the ice machine. Observed interior of ice machine with mold like substance dripping directly onto drink ice. See stop sale.
31A-15-4    Intermediate - Employee filled water pitcher/cup at handwash sink. Observed by servers ' station.
12A-03-4    Intermediate - Employee washed hands in a sink other than an approved handwash sink. Observed food prep worker washed hands in 3 compartments sink. Had Employee washed hands in handwash sink. **Corrected On-Site**
22-22-4    Intermediate - Encrusted material on can opener blade and base soiled. Operator is cleaning both at this time. **Corrective Action Taken**
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Observed employee washed wiping cloth in handwash sink in kitchen.
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Observed infront of cookline area..
