Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

10-07-4    Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Rice scoop in kitchen by cookline in standing water.
21-06-4    Basic - Paper towel used as liner for food container. Observed paper towels in container holding product on cook line reach in cooler. **Repeat Violation**
23-05-4    Basic - Soil residue build-up on nonfood-contact surface. Located on bulk containers in kitchen prep area. **Repeat Violation**
23-12-4    Basic - Walk-in cooler shelves soiled with encrusted food debris.
14-41-4    Basic - Walk-in cooler gasket torn/in disrepair.
23-10-4    Basic - Walk-in cooler gaskets soiled with slimy/mold-like build-up. **Repeat Violation**
14-17-4    Basic - Walk-in cooler shelves with rust that has pitted the surface. **Repeat Violation**
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Noodles at 48, chicken at 47, chicken employees stated the product has been held for more than 4 hours. **Repeat Violation**
01B-02-4    High Priority - Stop Sale issued on potentially hazardous (time/temperature control for safety) food due to temperature abuse. Noodles at 48, raw chicken at 47.
22-20-4    Intermediate - Accumulation of black/green mold-like substance in the interior of the ice machine. **Repeat Violation**
11-07-4    Intermediate - Certified Food Manager or person in charge lacks knowledge of foodborne illnesses and symptoms of illness that would prevent an employee from working with food, clean equipment and utensils, and single-service items.
22-05-4    Intermediate - Cutting board(s) stained/soiled.
16-36-4    Intermediate - No color/concentration comparison chart available for chemical test kit used to check sanitizer. Located on quat test strips.
53A-05-6    Intermediate - No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. 53B-10-4    Intermediate - Records/documents for required employee training do not contain all of the required information. Date of birth missing on Nick Sush, Trung Le Food Handler certificate.
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Located in kitchen by door exitto hibachi station.
