Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

Observed two dead roaches in corner of non used wait station/storage area off of dining room. Observed two dead roaches in water heater room. Water heater is located inside room inside men's rest room.
14-11-4    Basic - Equipment in poor repair. Door to walk in cooler. Frame of door is damaged with wood rot.
36-22-4    Basic - Floor area(s) covered with standing water. Observed standing water in walk in cooler. Overflow from condensation catch buckets. **Repeat Violation**
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed debris on floor in corners of bar area, under counters in bar area. Observed debris on floor along wall under dish area. Observed debris on floor in back corner of walk in cooler.
25-05-4    Basic - Single-service articles improperly stored. Bags of single service paper towels stored on floor in storage area next to dining room.
23-05-4    Basic - Soil residue build-up on nonfood-contact surface. Observed food debris/dust buildup on shelving in walk in cooler. Observed grease buildup on shelf under grill in kitchen area. Observe buildup of black mold like substance on wall behind dish area.
09-01-4    High Priority - Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Employee cutting fruit in kitchen area with bare hands. Employee placed on gloves for the remainder of prep. **Corrected On-Site**
50-17-2    High Priority - Operating with an expired Division of Hotels and Restaurants license. License expired June 1, 2018. **Admin Complaint**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed two droppings on floor in corner of non used wait station/storage area off of dining room. Observed three droppings in corner of water heater room. Water heater room is located inside men's restroom.
22-02-4    Intermediate - Accumulation of food debris/grease on food-contact surface. Observed buildup on slicer blade and slicer guard. Observe buildup on can opener blade. Also observed food debris buildup on bottom inside freezer in cooler in office/storage area.
22-05-4    Intermediate - Cutting board(s) stained/soiled. Cutting board on reach in cooler on cook line.
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Ice dumped in hand washing sink in bar area.
53B-14-5    Intermediate - Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. One certificate in date. All others provided are expired. **Repeat Violation**
