Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. **Corrected On-Site**
16-13-4    Basic - Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Observed employee washing utensils without soap and sanitation.
14-10-4    Basic - Equipment or utensils not designed or constructed in a durable manner. Observed a splash guard needed at hand wash sink next to cookline.
10-06-4    Basic - In-use utensil not stored with handle above the top of potentially hazardous (time/temperature control for safety) food and rim of the container. Observed scoop with handle stored in sugar container. **Corrected On-Site**
14-17-4    Basic - Walk-in cooler shelves with rust that has pitted the surface.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed sugar and seasoning removed from the original containers not identified by common names.
12A-10-4    High Priority - Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. 1. Observed cooked touch his nose with gloves and then handle ready to eat food without changing gloves and washing hands. 2. Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Female employee touching dirty utensils then touched clean plates. **Corrected On-Site** **Repeat Violation**
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed 1 live flying insect landing on wall in prep area.
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Observed beef cold holding at 47 degrees Fahrenheit in made shift table. Operator had instructed to place more ice in equipment. **Corrective Action Taken**
08A-05-4    High Priority - Raw animal food stored over ready-to-eat food. Observed raw chicken stored over mayonnaise in walk in cooler. **Corrected On-Site**
03A-06-4    High Priority - Raw meat/poultry cold held at greater than 41 degrees Fahrenheit. Observed chicken (46°F - Cold Holding); Beef (46°F - Cold Holding); Fish (47°F - Cold Holding) , operator transfer items to the walk in cooler. **Corrective Action Taken**
