Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance on cook line.
14-09-4    Basic - Cutting board has cut marks and is no longer cleanable. Observed grooved cutting board on prep table backroom. **Repeat Violation**
12B-12-4    Basic - Employee beverage container in a food preparation/clean dishware area does not have a lid and a straw or equivalent dispensing mechanism to prevent hand contact with lip contact area of drink container. Observed drink at sushi station no lid or straw.
36-41-4    Basic - Fan cover in walk-in cooler/freezer has accumulation of dust or debris. **Repeat Violation**
14-69-4    Basic - Ice buildup in reach-in freezer.
10-12-4    Basic - In-use ice scoop stored on soiled surface between uses. Observed ice scoop on soiled top of ice machine. **Corrected On-Site**
38-07-4    Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Lights shield in ware washing area missing. **Repeat Violation**
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Observed no sign at hand wash sink at bar.
29-49-5    Basic - Observed standing water in bottom of reach-in cooler on cook line.
33-16-4    Basic - Open dumpster lid.
23-05-4    Basic - Soil residue build-up on nonfood-contact surface. Observed soiled top of dishwasher. **Repeat Violation**

21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. **Repeat Violation**
21-38-4    Basic - Wiping cloth sanitizing solution stored on the floor. **Corrected On-Site**
08B-56-4    High Priority - Food stored in ice used for drinks. See stop sale. Observed sports drink bottle laying on top of ice in corner of ice machine. **Corrected On-Site**
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Observed on cook line reach in cooler sprouts (58°F - Cold Holding) **Corrective Action Taken**
12A-03-4    Intermediate - Employee washed hands in a sink other than an approved handwash sink. Observed employee rinsing hands in three compartment sink at sushi bar.
02B-01-4    Intermediate - Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Lunch menu does not identify which items may be served raw or undercooked., ex: sushi rolls.
