A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

12A-27-4    High Priority - Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Cook cracked a raw shell egg and then grabbed a hand full of uncooked mushroom without changing his gloves and washing his hands. Reviewed proper handwash procedure with manager. **Warning**
12A-10-4    High Priority - Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed cook preparing fried rice for an order and saw him grabbing some of that rice with his hands and eat it. Observed cook touched his lips when eating the rice and then handled uncooked mushrooms without washing his hands and changing his gloves. Reviewed proper hand wash procedure cook and manager. Asked cook to washed hands but he only rise hands with water, he did not use soap nor scrubbed hands for at least 10 seconds. **Warning**
12A-20-4    High Priority - Employee washed hands with no soap. Cook did not used soap when washing his hands after touching his mouth and then proceeded to handle food. Reviewed proper handwash procedures with manager. **Warning**
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Three flying insects on cook line near dish machine. **Warning**
03B-01-5    High Priority - Potentially hazardous (time/temperature control for safety) food hot held at less than 135 degrees Fahrenheit or above. cooked white rice (112-114°F) in a pan at room temperature on cook line. Manager voluntarily discarded cooked rice. **Corrected On-Site** **Warning**
22-20-4    Intermediate - Accumulation of black/green mold-like substance in the interior of the ice machine. **Warning**

12A-03-4    Intermediate - Employee washed hands in a sink other than an approved handwash sink. Observed server washed his hands at the three compartment sink and then proceeded to cooked and handled food on cook line. Reviewed proper handwash procedure with manager. **Warning**
27-23-4    Intermediate - Hot water at handwash sink does not reach 100 degrees Fahrenheit. Hot water at hand wash sink next to dish washing machine only goes up to 94°F. **Warning**
22-28-4    Intermediate - Interior of reach-in cooler soiled with accumulation of food residue. Interior of non working unused refrigerators soiled with food residue on cook line. **Warning**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. No paper towels provided at the handwash sink near the mop area. Manager provided paper towels. **Corrected On-Site** **Warning**
