Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

14-01-4    Basic - Bowl or other container with no handle used to dispense food. Bowl used as scoop in salt, in container in back area. Handleless Pan used as scoop in beans in back storage area. **Repeat Violation**
29-24-4    Basic - Clean-out cap missing from plumbing line. Clean out cap on exterior of building missing. Pipe running from wall where prep sink and hand washing sink in prep area are located. Also, cleanout cap close to front entrance is laying at an angle on top of pipe, not connected.
32-12-5    Basic - Covered waste receptacle not provided in women's bathroom. Missing in women's restroom.
12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee drink without lid on prep table in prep area. **Repeat Violation**
40-07-4    Basic - Employee personal items stored with or above food, clean equipment and utensils, or single-service items. Employee purse stored in top of bag in box soda in dish area. Used apron stored on top of beer in back storage area. **Repeat Violation**
36-73-4    Basic - Floor soiled/has accumulation of debris. Food Debris observed under prep table in prep area. Food debris observed along walls and in corners under shelving in dry storage area and ice machine. Food debris and grease observed under cooking equipment. **Repeat Violation**
08B-12-4    Basic - Food stored in holding unit not covered. Cut onions and peppers in bowl on cook line. Taco shells in prep area. Dried beans in dry storage area. Dried rice in dry storage area. Flies observed in all areas noted.
08B-38-4    Basic - Food stored on floor. Cases of fryer oil stored on floor in dry storage shed.
33-19-4    Basic - Garbage on the ground and/or pad around dumpster. Observed debris scatter around parking lot. Discarded cloth towel, lids near back door. Large pile of cat food scatter in corner of parking lot near dumpster area.
36-47-5    Basic - Hood soiled with accumulated grease, dust or food debris. Observed debris on side of hood system. Observed buildup of grease on hood filters **Repeat Violation**
38-07-4    Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Missing on lights in prep area.
35B-03-4    Basic - Outer openings not protected with self-closing doors. Back door. Neither door self closing.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Wet wiping cloth in prep area on prep table. Also, In-use wet wiping cloth/towel used under cutting board in same area.
12A-14-4    High Priority - Employee coughed, sneezed, used a handkerchief or disposable tissue, used tobacco, ate, or drank then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee in prep area blew nose, placed on gloves and began working with food. Inspector stopped employee, discussed hand washing. Employee washed hands.
22-54-4    Basic - High Priority - Equipment and utensils rinsed after the application of the sanitizer solution. Employee at three compartment sink washed pan, dipped into chlorine sanitizer then rinsed Pan. Dissuaded proper ware washing procedures with employee and manager. Manager had employee dump sink and refill.
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed 6-7 live flies in kitchen and prep area. Observed 3 live flies inside of storage area outside of back door. During inspection observed multiple flies land on bowl of cut onions and peppers stored on rack in kitchen area.
08A-20-4    High Priority - Raw animal foods not properly separated from each other in holding unit. Raw chicken stored above beef in three door reach in cooler in prep area. Raw animal foods not properly separated from one another based upon minimum required cooking temperature in reach-in freezer - all products not commercially packaged. Raw chicken stored above raw beef in reach in freezer in prep area.
28-13-4    High Priority - Raw sewage on ground of establishment. Observed waste water flowing out of pipe at cleanout caps. One clean cleanout cap is located in parking lot near front door. One clean out cap is located at corner of building and parking lot. Appears that waste water overflowed from outdoor plumbing when three compartment sink was drained during inspection. Observed food debris and grease mixed with water.
01B-03-4    High Priority - Stop Sale issued due to contamination of food product. Observed multiple flies land on bowl of cut onions and peppers.
41-10-4    High Priority - Toxic substance/chemical improperly stored. Bottle of Fabuloso cleaner stored on shelf above steam table in back prep area. Hot shot flying insect spray stored on prep table with employee prepping chicken.
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Missing on both faucets on prep sink in prep area.
22-22-4    Intermediate - Encrusted material on can opener blade. Food debris on can opener blade, also on sides near blade.
02C-02-4    Intermediate - Ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked. Beans, pork in reach in cooler. Items prepared yesterday per employee **Repeat Violation**
