Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

25-25-4    Basic - Box containing roll plastic wrap has old food debris/soil residue inside on storage shelf in dry storage area.
23-06-4    Basic - Build-up of food debris, dust or dirt on nonfood-contact surface. - Observed grease buildup on wheels of equipment in cook line. - Observed food debris buildup on shelving in dry storage area. - Observed buildup of bread crumbs on bun tray holder.
35A-03-4    Basic - Dead roaches on premises. Observed 1 dead roach in dry storage area by drive through window and 1 dead roach on floor near ice machine.
36-71-4    Basic - Floor drains/drain covers heavily soiled. Observed soiled floor drains under three compartment sink.
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed floors along the walls in kitchen, dry storage, and self service area.
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed 5 small flying insects in dry storage area and food prep area.
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Observed cut lettuce 57°F - 42°F in small cooler by cook line. Cooler unplugged, manager moved food to walk in cooler and plugged in unit. **Corrected On-Site**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed 7 live roaches behind reach in cooler between hand wash sink and three compartment sink on floor and wall.
