Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

33-12-4    Basic - Dumpster rusted out on bottom. Observed what appears to be rust damage on front right of dumpster. Observed leakage from under dumpster onto ground next to dumpster.
36-73-4    Basic - Floor soiled/has accumulation of debris. Carpet in hallway leading to restroom has and excessive about of buildup along edges. Area around corner on hallway and into kitchen area has buildup on floor and along cove molding.
23-05-4    Basic - Soil residue build-up on nonfood-contact surface. Buildup of debris observed on pipe under and beside three compartment sink. Buildup of debris on area near hinge of ice machine. Buildup of debris on top/back edge of dish machine.
12A-07-4    High Priority - Employee failed to wash hands before changing gloves and/or putting on gloves to work with food. Employee walked into kitchen area. Placed no glove to cut limes but did not properly wash hands. Inspector discussed with employee.
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed 8-9 small flies around dirty dish cart in hallway leading to restrooms. Observed 3 small live flies in hallway in front of men's rests room, near back entrance to dish area. Observed 2 small live flies in area next to dish machine. Person in Charge began cleaning area with spray bleach and attempting to eliminate issue. **Corrective Action Taken** **Repeat Violation**
03A-02-4    High Priority - Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. Chicken 54°. Shrimp 54°. Items in reach in cooler on cook line. Cooked beans cooling directly below items. Appears that when items were placed into cooler approximately 3 hours ago, it facilitated a temperature rise in other food items.
08A-05-4    High Priority - Raw animal food stored over ready-to-eat food. Raw chicken, raw shrimp stored in shelf above salsa, cut lettuce, beans and cheese in double door reach in cooler on cook line. **Repeat Violation**
53A-05-6    Intermediate - No currently certified food service manager on duty with four or more employees engaged in food preparation/handling.
