Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

Observed eight dead roaches in establishment: two on floor below bag in box soda rack in dry storage of kitchen, one in walk basin of triple sink in dish area, one on floor under spray hose sink in dish area, one on floor in dry storage area under rack in kitchen, two by exit door on south side of building near restrooms, and one on floor by entrance to kitchen near restrooms. 36-02-5    Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Near utility sink by hot water room.
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Under spray hose sink in dish area. **Repeat Violation**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Sugar in plastic container in server station in kitchen. Container labeled. **Corrected On-Site**
12A-04-5    High Priority - Employee handled soiled dishes or utensils and then handled clean dishes or utensils without washing hands. Employee with dirty dishes in both hands, transferred the dishes to one hand, handled clean cups and dispensed iced tea. Manager coached employee and she washed her hands. **Corrective Action Taken**
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Observed approximately 26 live flies in establishment: three in kitchen area near expo/ cook line, two in area near ice machine in kitchen area, eighteen in bar area under beer tap, one near front window near front door, and one in dining area near restrooms. Fourteen flies in bar area were killed and discarded. **Corrective Action Taken**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed two live roaches at establishment: one in empty ice cream cooler in kitchen by server area and one in small service station in dining room near front door. Both roaches killed and discarded. **Corrective Action Taken**
41-08-4    High Priority - Toxic substance/chemical stored by or with clean or in-use utensils. Can of insecticide by clean utensils in dry storage area. Can removed. **Corrected On-Site**
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Missing on faucet for hand washing sink and prep sink in prep area.
02C-03-4    Intermediate - Commercially processed ready-to-eat, potentially hazardous (time/temperature control for safety) food opened and held more than 24 hours not properly date marked after opening. Milk container in walk in cooler opened yesterday not dated. Manager marked item. **Corrected On-Site**
