Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

08B-44-4    Basic - Case/container/bag of food stored on floor in kitchen. Case of broccoli. Operator removed from floor. **Corrected On-Site**
28-02-4    Basic - Condensation not disposed of according to law. Condensation from AC vents in dining room dripping onto to tables and floor.
36-03-4    Basic - Cove molding at floor/wall juncture broken/missing. Throughout entire kitchen. Floor underneath of buffet in disrepair. Rotting wood inside buffet. Floor tiles in kitchen and dining room area cracked. Hole in or other damage to wall. Throughout entire kitchen and dining area. Light not functioning. In dessert prep area. Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. **Repeat Violation**
35A-03-4    Basic - Dead roaches on premises. Approximately 30 dead inside buffet. 2 dead by soda boxes in kitchen. 5 dead on chemical dry storage shelving. 1 dead in back of prep cooler.
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Exit door in dining room.
10-07-4    Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Ice cream scoops and rice spoon. Operator removed. **Corrected On-Site**
36-70-4    Basic - Objectionable odor in establishment.
12B-13-4    Basic - Opened employee beverage container in a cold holding unit with food to be served to customers. Operator removed from walk in cooler.
29-11-4    Basic - Water leaking from faucet at hand wash sink in prep area. Drain cover(s) missing. In kitchen.
36-10-4    Basic - Worn, torn and/or soiled floors/carpeting. In dining room. Standing water observed in food containers used to catch water under sushi buffet. Standing water in kitchen under three compartment sinks and dishwashing area. Floor drains/drain covers heavily soiled. **Repeat Violation**
35A-02-5    High Priority - Live, small flying insects in kitchen, food preparation area, or food storage area. Approximately 15 under buffet. Approximately 25 inside cabinet under buffet.
08A-01-4    High Priority - Raw animal food stored over cooked food. Raw chicken over cooked noodles. Operator rearranged so that raw chicken was under cooked noodles. **Corrected On-Site**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 1 live in hot plate holder in buffet. 2 live inside buffet. 1 live under three compartment sink in prep area 1 live by hand wash sink in prep area.
35A-23-4    High Priority - Roach excrement and/or droppings present. On dishes being held inside of broken cooler.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. 10 droppings found under buffet. Approximately 15 droppings under buffet. Approximately 15 inside non working walk in cooler used for storage. 3 on floor under shelving in dry storage.
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. No soap provided at handwash sink. No handwashing sign provided at a hand sink used by food employees. In employee restroom. Operator placed paper and soap in restroom. **Corrected On-Site**
