Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

14-71-4    Basic - Duct tape used to repair nonfood-contact surface. Observed at front reach in freezer. Duct tape used to fix glass panel. **Repeat Violation**
13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Cooks.
36-24-5    Basic - Hole in or other damage to wall. Unisex restroom on the west side and above the kitchen sink.
36-47-5    Basic - Hood soiled with accumulated grease, dust or food debris. .
14-69-4    Basic - Ice buildup in reach-in freezer. Chest freezer kitchen and front counter glass freezer.
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Front counter and restrooms. Provided form to operator.
33-34-4    Basic - Storage area not maintained clean and organized. By the kitchen door.
33-31-5    Basic - Unnecessary items/unused equipment on the premises. Kitchen two non working fryers in the kitchen.
36-26-4    Basic - Wall soiled with accumulated black debris in dishwashing area.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Throughout the kitchen.
29-11-4    Basic - Water leaking from faucet/handle at the three compartment sink.
12A-20-4    High Priority - Employee washed hands with no soap. Observed employee washing hands in the kitchen hand sink with no soap. Inspector coached employee. Operator placed soap pump at the sink. Employee washed hand and new placed gloves on. **Corrected On-Site**

32-11-5    Intermediate - Lack of toilet tissue at each toilet.
16-35-4    Intermediate - No chlorine chemical test kit provided when using chlorine sanitizer at three-compartment sink/warewashing machine. **Repeat Violation**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Kitchen. **Corrected On-Site**
05-08-4    Intermediate - No probe thermometer provided to measure temperature of food products. **Repeat Violation**
31B-03-4    Intermediate - No soap provided at handwash sink. Observed at the kitchen hand washing sink. Operator sent employee to buy more soap during the inspection. **Corrected On-Site**
