Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

06-05-5    Basic - Time/temperature control for safety food thawed in standing water. Observed calamari thawing in standing water.
36-02-5    Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Kitchen area.
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Observed a leaking pipe in the hand sink located in the kitchen area.
21-07-4    Basic - Wiping cloth chlorine sanitizing solution not at proper minimum strength.
09-01-4    High Priority - Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee handling cheese with bare hand contact in the kitchen area, coached operator in hand washing procedures, employee washed hands and used gloves. **Corrected On-Site**
14-15-4    High Priority - Nonfood-grade containers used for food storage - direct contact with food. Observed flour stored in a non food grade container in the kitchen area.
08A-05-6    High Priority - Raw animal food stored over ready-to-eat food. Observed raw shell eggs stored over cooked lasagna in the walk in cooler.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed fried artichoke (51°F - Cold Holding) stored at room temperature on top of prep table as per operator less than 30 minutes, operator placed food in the reach in cooler. **Corrective Action Taken**
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed hose connected wit no vacuum breaker by the dishwasher area.
29-42-4    High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. **Repeat Violation**
41-27-4    High Priority - Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed Sanitizer Bucket (Chlorine 200ppm) on top of the kitchen.
22-24-5    Intermediate - Accumulation of black/green mold-like substance on/coffee machine.
01C-03-4    Intermediate - Clam/mussel/oyster tags not marked with last date served. **Repeat Violation**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink located by the desserts cooler.
51-16-7    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Observed operator added an extension to the back of the business as storage that has an ice machine, mop sink, shelves with pans and pots, tomatoes can sauce, single service items, boxes of liquid fried oil and unnecessary items.
