Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

14-01-5    Basic - Bowl or other container with no handle used to dispense food. Observed throughout the kitchen from the server line to cook line, and the walk in cooler. Operator removed the no handle containers from the food containers. Ice buildup in walk-in freezer. Reach-in cooler gasket torn/in disrepair. Observed under grill drawers with torn gaskets. Walk-in cooler/freezer shelves with rust that has pitted the surface. **Corrected On-Site**
24-05-4    Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed food plates and bowls at sever station, cook line, and dish draining rack not inverted.
25-10-4    Basic - Coffee filters not stored in a protected manner to prevent contamination. Observed coffee filter not stored in a protected manner at server station. Operator stored properly. **Corrected On-Site**
14-10-4    Basic - Equipment or utensils not designed or constructed in a durable manner. Observed spatulas that were burnt and no longer fit for use as the burnt portion disintegrates when touched.
36-22-4    Basic - Floor area(s) covered with standing water. Observed standing water on dry storage floor.
36-11-4    Basic - Floors not maintained smooth and durable. At ware washing area. Floor soiled/has accumulation of debris. Observed debris behind oven at cook line. Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Throughout.
10-08-5    Basic - Ice scoop handle in contact with ice. Observed ice scoop handle in ice in ice machine. Operator removed the ice scoop and stored properly. **Corrected On-Site**
10-01-5    Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed handle of scoop in contact with sugar in dry storage containers and in bag of sugar in walk in cooler. Operator stored scoop in the upright position. **Corrected On-Site**
51-13-4    Basic - No Heimlich maneuver/choking sign posted, e-mailed another copy to operator. **Corrected On-Site** **Repeat Violation**

21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed wet wiping cloth in several areas on shelf under prep table at server line, cook line and not stored in sanitizing solution. **Corrected On-Site**
08B-56-4    High Priority - Food stored in ice used for drinks. See stop sale. Observed a can of soda stored in drink ice in ice machine. Uncovered food stored near sink exposed to splash. Observed a round lid on a square container with corners of the container open with bread partially exposed next to the hand wash sink at the cook line. Operator placed the square lid on the container.
10-05-5    High Priority - In-use utensil stored in unclean water at or above 135 degrees Fahrenheit. Observed ice scoop stored in dirty water at server station. Operator replaced scoop and container in which it was stored.
08A-05-6    High Priority - Raw animal food stored over ready-to-eat food. Observed raw steak over case of bottled water in walk in cooler. Operator stored raw steak properly. **Corrected On-Site**
12A-02-4    High Priority - Server handled soiled dishes or utensils and then picked up plated food, served food, or prepared a beverage without washing hands. Observed servers handling dirty plates then touch condiments and clean plates. Operator advised to encourage hand washing after touching soiled or dirty plates before touching food or clean plates.
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Food stored in ice used for drinks. See stop sale. Observed a can of soda stored in drink ice in ice machine.
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Wait station- strawberry butter- 51, whipped butter 64, milk 55-56, salsa 59 placed in cooler yesterday. Drawers under flat top grill- hash brown 54, sliced ham 52, corned beef hash 49, shredded cheddar 52, diced ham 48, shredded Swiss cheese 52, cooked turkey sausage 47, cold smoked salmon 56. All foods placed in cooler at about 6:30-7:00 am today. First temperature taken between 10:00-10:15 am. Second temperature check at about 12:15, all TCS foods between 48-56°F. Salad station- cut tomato 46-47° cut yesterday, all batters- 48-55- prepped yesterday. First temperature taken between 10:00-10:15 am. 03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. 03D-05-5    High Priority - Time/temperature control for safety food cooked/heated yesterday/on a previous day not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within a total of 6 hours. Food has been cooling overnight. Observed hash brown cooked yesterday in drawer under grill at cook line at 71°F at 12:30 pm.
03F-02-5    High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Per operator, time as a public health control is used for hollandaise sauce. Sauce was placed under time at 7:00 am. Observed at 10:00 am. Stop sale was not issued as sauce had 1 more hour under time. No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Did not observe a written plan. Printed a copy for operator.
41-09-4    High Priority - Toxic substance/chemical stored in food preparation area with potential to cause cross contamination. Observed sanitizing solution in a spray bottle stored on a food prep table in the back of the kitchen.
22-20-5    Basic - Intermediate - Accumulation of black/green mold-like substance in the interior of the ice machine. Repeat Violation. Interior of oven has heavy accumulation of black substance/grease/food debris. Soil residue in food storage containers. Observed soiled food storage containers in the dry storage area and in walk in cooler. Encrusted material on can opener blade. Cutting board(s) stained/soiled. Interior of reach-in cooler soiled with accumulation of food residue. Cook line. Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed chlorine at 10 ppm on three cycles. Operator advised to set up three compartment sink with quaternary at about 200 ppm for sanitizing. **Repeat Violation**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Did not observe paper towels at several hand wash sinks. No soap provided at handwash sink. Did not observe soap at two hand wash sink. Soap dispenser at handwash sink not working/unable to dispense soap. At dish machine area.
05-08-4    Intermediate - No probe thermometer provided to measure temperature of food products. Observed thermometer that checks only food surface.
53B-01-5    Intermediate - No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. **Warning**
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed several TCS food throughout the kitchen cooked 24 hours or more without date marking. Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed milk opened yesterday in 2 reach in cooler without date marking.
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Observed glass cleaner in unlabeled spray bottles.
