Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. -Observed employee cigarettes stored on shelf above make table in kitchen. -Observed employee cell phone stored on boxes in dry storage area in kitchen. -Employee keys stored on food preparation table in kitchen.

08B-38-4    Basic - Food stored on floor. -Observed buckets of soy sauce and cooking oil stored on floor throughout kitchen and walk in cooler. -Observes bag of onions and bucket holding raw chicken breast stored on in walk in cooler.

10-01-5    Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. -Observed spoon handle stored in white rice hot holding unit in kitchen.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. -Walk-in cooler gaskets soiled with slimy/mold-like build-up. -Food debris/dust/grease/soil residue on exterior of oven and cook line. -Food debris/dust/grease/soil residue on exterior of oven and cook line in kitchen. **Repeat Violation**
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. -Observed Cook Line make Table shelves soiled with food debris in kitchen. **Repeat Violation**
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. -Observed to-go containers stored above cook line make table not inverted/protected **Repeat Violation**
08B-12-5    Basic - Stored food not covered. -Observed containers holding cooked egg rolls, rice, general chicken, sweet and sour chicken, raw beef, raw pork and chicken stored in walk in cooler. -Observed containers holding raw broccoli and cut cabbage stored on prep table next to cook line uncovered. **Repeat Violation**
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. -Observed precooked pork thawing on food preparation table in kitchen.
14-17-4    Basic - Walk-in cooler shelves with rust that has pitted the surface. **Repeat Violation**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. -Observed 1 live fly in kitchen food preparation area. Instructed operator to remove fly immediately.
41-10-4    High Priority - Toxic substance/chemical improperly stored. -Observed bottles of Isopropyl Alcohol stored on shelf next to canned pineapple juice in kitchen.
16-33-4    Intermediate - Chemical test kit not used to ensure proper sanitization of equipment and utensils when using a chemical sanitizer. -No chlorine test strips provided at time of Inspection.
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. -Observed plastic jug and large spoon stored in hand wash sink in food preparation area in kitchen. -Observed front counter hand wash sink blocked by cases of water and canned juice in kitchen.
