A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Dead roaches on premises.
Observed approximately 4 dead roaches in hallway near bathroom. Operator discarded and sanitized.
Observed approximately 7 dead roaches on floor next to chest freezer. Advised operator to discard and sanitize area.
Observed approximately 7 dead roaches on floor by ice machine.
Observed approximately 2 dead roaches on floor next to front counter triple sink. **Warning**
Basic - No handwashing sign provided at a hand sink used by food employees. Observed at both hand sinks in kitchen. Emailed and printed for operator to post. **Corrected On-Site** **Warning**
Basic - Water draining onto floor surface. Observed leak in unisex bathroom when faucet is running. Operator stated work order in for repairs. **Corrective Action Taken** **Warning**
High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed container of raw fish stored over cooked octopus and other ready to eat foods in reach in cooler near chest freezer. Advised operator to rearrange. Operator removed and stored properly. **Corrected On-Site** **Warning**
High Priority - Roach activity present as evidenced by live roaches found. Observed 3 live roaches on kitchen floor crawling next to triple sink.
Observed 1 live roach crawling on wall near ice bin. Observed 2 live roaches crawling on and behind chest freezer. **Warning**
High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Observed rack of cheese pizzas with no time stamp at 4:22pm. Per operator, placed out at 3pm. Operator placed time stamp and will discard at 7pm. **Corrected On-Site** **Warning**
Intermediate - 1.) Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed hot water handle not functioning at hand sink in kitchen next to triple sink.
2.) Observed hot water at hand sink in unisex bathroom at 79°F. **Repeat Violation** **Warning**
Intermediate - No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. **Warning**
Intermediate - No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Printed and emailed form to operator. Assisted operator in completing paperwork. **Corrected On-Site** **Warning**
