Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

08B-49-4    Basic - Employee personal food not properly identified and segregated from food to be served to the public. Observed employee frozen pig feet inside the chest freezer. Employee removed food. **Corrected On-Site**
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. **Repeat Violation**
13-03-4    Basic - Employees with no hair restraint while engaging in food preparation.
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed walk in cooler has accumulation of old food debris, and liquid substance.
14-69-4    Basic - Ice buildup in reach-in freezer.
10-20-4    Basic - In-use tongs stored on oven door handle between uses.
22-08-4    Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris.
22-13-5    Basic - Juice dispenser located at the front counter is not cleaned at least once every 24 hours. Observed mold like substance inside the juice dispenser.

23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed exterior of stove, grill, deep fryer, and ice machine has accumulation of grease, dust and dirt,
08B-47-4    Basic - Observed potatoes not stored at least 6 inches off of the walk in floor.
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Observed both soda reach in coolers has dead insects and dirt inside. **Repeat Violation**
14-20-4    Basic - Ripped/worn tin foil used as shelf cover. Observed on shelves in the walk in cooler.
25-05-4    Basic - Single-service articles improperly stored. Observed packages (sleeves) of cups and lid covers are stored on the walk in cooler floor.
36-06-4    Basic - Soiled Cardboard used on kitchen floor as anti-slip measure not replaced every day or when heavily soiled, whichever comes first.
22-37-4    Basic - Three-compartment sink used for warewashing not cleaned before use as a food preparation sink. Observed old food debris inside the sink which has a package of steak.
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. Observed a package of beef in the three compartment sink being thawed out with no running water.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Throughout the kitchen areas. **Repeat Violation**
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry. Observed 2 mops not inverted. **Repeat Violation**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed a jar of coffee not labeled at the front counter.
12A-20-4    High Priority - Employee washed hands with no soap. Employee was observed washing hands with no soap. Employee was instructed to wash hands. Employees washed hands. **Corrected On-Site**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed both soda reach in coolers has dead insects and dirt inside.
14-31-5    High Priority - Nonfood-grade bags used in direct contact with food. Observed nonfood grade bag indirect contact with pig feet inside the chest freezer.
50-17-2    High Priority - Operating with an expired Division of Hotels and Restaurants license. Observed establishment is open to the public and serving customers, with an expired Division of Hotels and Restaurants license. Management was encouraged to pay DBPR Business License. **Admin Complaint**
08A-20-5    High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw chicken over raw steak, and vegetables in the kitchen reach in cooler. Employee placed raw chicken at the bottom shelf. **Corrected On-Site**
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Observed fried tortillas in a plastic bowl on the preparation table is covered with dirty dish cloth.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed cut lettuce and tomatoes in a bowl at 78°F. Employee discarded all items. **Corrected On-Site**
41-10-4    High Priority - Toxic substance/chemical improperly stored. Observed a bottle of degreaser on a shelf with clean pans. Located above the three compartment sink. Observed a bottle of windex, and alcohol on a shelf with utensils. **Repeat Violation**
29-42-4    High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. **Repeat Violation**
12A-03-4    Intermediate - Employee washed hands in a sink other than an approved handwash sink. Observed employees washing hands in the kitchen three compartment sink. **Repeat Violation**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed multiple cutting boards are soiled. **Repeat Violation**
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Observed mop bucket in the front counter blocking handwash sink. Observed mop and a chair in the kitchen that is blocking handwash sink.observed fish liquid in the kitchen handwash sink, and a sponge in the front counter handwash sink. **Repeat Violation**
