Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed drink no lid above dishwasher machine.
13-07-4    Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed operator engaging food preparation with personal bracelet. Operator removed personal bracelet. **Corrected On-Site**
14-11-4    Basic - Equipment in poor repair. Observed cooling unit in disrepair at seafood market, no item stored inside.
06-06-5    Basic - Frozen time/temperature control for safety food being slacked at room temperature is no longer frozen solid. Observed fries slacked at room temperature no longer frozen. Operator discarded. **Corrected On-Site**
33-10-4    Basic - Inside/outside of dumpster not clean. Observed strong fishy smell and standing water inside and around dumpster area.

21-38-4    Basic - Wiping cloth sanitizing solution stored on the floor. Observed two buckets stored at kitchen area.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed unknown condiment not labeled above mixer at preparation area.
12A-18-4    High Priority - Employee dried hands on clothes/apron/soiled towel after washing. Observed employee drying hand multiple times on dirty appron.
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed approximately 12 live flies located around trash can and dirty plates at tiki bar , 2 live flies above service area, 5 live flies at ceiling on the kitchen area, and 1 live fly above bread, 1 live fly landed on salad, 3 above condiments squeeze bottles.
