Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

06-09-1    Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed mahi mahi thawed stored in walk in cooler still in ROP packaging. **Warning**
12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee beverage on next to coffee machine at front counter at bar area. Operator had employee remove cup and store correctly **Corrected On-Site** **Warning**

35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Observed gap beneath exterior back door. **Warning**
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed floors soiled with build up food debris behind equipment and reach in coolers located at bar area. **Warning**
35B-08-4    Basic - Fly sticky tape hanging over food/food preparation area/food-contact equipment. Observed fly trap device with pro glue board sticky device with 12 dead flies next to hand washing sink located at front counter bar area. **Warning**
10-07-4    Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed ice cream scoop in standing water on reach in cooler located in kitchen area. **Warning**
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Observed reach in coolers located at bar area storing bottled waters and wine bottles with build up mold like substance. **Warning**
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Observed plastic drinking cups stored in holder located at bar area ice machine not inverted. Operator removed cups from holder and stored correctly. **Corrected On-Site** **Warning**

35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed approximately 11 live flies flying around at bar area located in front area by hand washing sink landing on single service wrapped straws, paper towel and paper cone covers covering alcohol bottles. Observed one live fly in reach in cooler located at bar area with bottled waters. Observed one live fly flying around in dry storage area room separate from food area not landing on any dry storage products. Operator able to kill nine flies started cleaning and sanitizing area **Warning**
08A-20-5    High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw chicken stored over raw fish in walk in freezer. Operator removed and stored correctly **Corrected On-Site** **Warning**

02C-08-5    Intermediate - Ready-to-eat, time/temperature control for safety food opened/prepared, frozen and then thawed and held at refrigeration temperature not properly date marked. Observed all thawed meats beef, steak, salmon, filet mignon, New York steak, raw shrimp, all defrosted in walk in cooler without any date marking. **Warning**
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed lasagna, bread pudding, chicken wings, made on March 6th not labeled. Operator labeled products Corrected on site. Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening.
