Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

23-24-4    Basic - Buildup of food debris/soil residue on equipment door handles. Observed door handles of reach in coolers at front soiled.
36-32-5    Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed tiles and air vents soiled throughout the kitchen area, as well as front of establishment.
14-09-4    Basic - Cutting board has cut marks and is no longer cleanable. Observed cutting boards grooved and soiled in prep table kitchen area.
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee phone stored above prep station. Employee removed the item. **Corrected On-Site** **Repeat Violation**

05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit. Observed no ambient thermometer in reach in coolers of establishment.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed fan blades soiled in kitchen area.

06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. Observed Raw meat thawing at ambient temperature in three compartment sink. As per person in charge less than 1/2 hour. Person in charge opened water faucet. **Corrected On-Site**
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed wet soiled towels not stored in sanitizer water at kitchen next to rice cooker.
14-31-5    High Priority - Nonfood-grade bags used in direct contact with food. Observed meat and tortillas stored in non food grade bags at front reach in cooler.
08A-04-5    High Priority - Raw animal food stored over or with unwashed produce. Observed raw chicken stored over not washed vegetables at kitchen reach in cooler. Person in charge removed the chicken. **Corrective Action Taken**
08A-20-5    High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw beef stored under raw chicken in reach in cooler in the kitchen. Person in charge reposition product. **Corrective Action Taken**
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature. Observed tamales (51 F cold holding) in reach in cooler.As per person in charge since yesterday afternoon. Person in charge discarded the product. **Repeat Violation**
03B-01-6    High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed cooked beef chunks (68 F - Hot Holding), breaded chicken (116 F - Hot Holding); chicken grilled (106F - Hot Holding), at steam table, as per manager for less than one hour, she took them back to reheat at stove. Beef chunks (209°F reheated), breaded chicken (168F reheated), Grill chicken (167° F reheated). .**Corrected On-Site** **Corrected On-Site**
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed no vacuum breaker at Bibb hose next to hand washing sink by three compartment sink.
11-07-5    Intermediate - Certified Food Manager or person in charge lacks knowledge of foodborne illnesses and symptoms of illness that would prevent an employee from working with food, clean equipment and utensils, and single-service items. Asked person I charge of possible consequences with improper thawing food or temperature violations as it relates to food borne illnesses. He was not able to answer.
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Observed hand washing sink blocked by a food cart next to three compartment sink. Person in charge removed the cart. **Corrected On-Site**
22-31-4    Intermediate - Non-pitting surface rust on food-contact equipment. Observed rusted rack under prep tables in front counter.
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed containers of marinated beef, in reach cooler pork not dated. As per manager since yesterday morning.
