Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

22-20-5    Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed interior of both ice machine lid, interior of ice machine and ice chute with mold like substance build up.
14-01-5    Basic - Bowl or other container with no handle used to dispense food. Observed no. Handle scoop used to dispense ice at bar area.
23-24-4    Basic - Buildup of food debris/soil residue on equipment door handles.
36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling tiles above grills with grease dripping off ceiling .
24-06-4    Basic - Clean utensils or equipment stored in dirty drawer or rack. Observed clean. Glasses stored on counter with mesh which has heavy mold like substance and slime like substance build up touching food contact surface of glass
24-14-4    Basic - Clean utensils stored between equipment and wall. Observed spatulas and scrapers stored in between reach in cooler and Grill.
50-09-4    Basic - Current Hotel and Restaurant license not displayed.
14-71-4    Basic - Duct tape used to repair nonfood-contact surface. Observed duct tape use to hold broken side of hand wash sink at bar area.
12B-02-4    Basic - Employee eating in a food preparation or other restricted area. Observed employee bar tender eating and drink at bar area while getting beverages for guests.
12B-10-4    Basic - Employee smoking in a food preparation, storage or warewashing area. Observed cigarette butt on table with sauces and to go soufflé cups.
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside.
36-73-4    Basic - Floor soiled/has accumulation of debris.

35B-08-4    Basic - Observed Fly sticky tape hanging over food/food preparation area/food-contact equipment, hand washing sink with approximately 20 or more dead flies attached to device
23-03-4    Basic - Observed Nonfood-contact surfaces b fryers under fryers, side of fryers, oil and slime build up on floor soiled with grease, food debris, dirt, slime from under 3 compartment sink throughout to standing reach in cooler

22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Observed beer reach in cooler sliding doors, interior of gaskets with heavy mold slime like build up substance.
29-49-6    Basic - Standing water in bottom of reach-in-cooler. Observed standing water in reach in cooler located in the kitchen area.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust.
41-07-4    High Priority - Container of medicine improperly stored. Observed medicine container stored on counter top at bar area and cough medicine in kitchen area.
12A-05-4    High Priority - Employee used tobacco, ate, or drank then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee eating and drinking meal at bar area, returned to serving guests without washing hands.
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 5 or more live flies flying around garbage bin area located at bar area. Observed 1 live fly landing on hand towel sink at kitchen area.
41-05-4    High Priority - Observed. Numerous Pesticide/insecticide labeled for household use only present in establishment located in bar area.
50-17-2    High Priority - Operating with an expired Division of Hotels and Restaurants license.
28-26-5    High Priority - Sewage/wastewater backing up through floor drains. Observed gray water coming from pipe beside standing reach in cooler, water is pouring on floor by running under grill area and under mat where employees are accessible to stand on water coming through mat.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed Tartar sauce opened 12/28; (held 46F - Cold Holding)
03B-01-6    High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed melted cheese in can held at 81F)
11-07-5    Intermediate - Certified Food Manager or person in charge lacks knowledge of foodborne illnesses and symptoms of illness that would prevent an employee from working with food, clean equipment and utensils, and single-service items.
02C-03-5    Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed half and half,horseradish sauce, sour cream opened more than 24 hours not date marked.
11-27-4    Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed soda nozzles gun at bar area with mold like. Substance and slime build up. Observed soda gun nozzle holders with built up mold like substance on the interior of holders .
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Observed hand washing sink located in kitchen area inaccessible with scrub pads,drain cover inside the sink.
53A-01-7    Intermediate - Manager or person in charge lacking proof of food manager certification.
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Observed bleach bottle not labeled by bar area.
