Below, is a link to a printable version of the filed complaint. In order to make a legal evaluation, any and all supporting documentation regarding the allegations must be in the Division's possession. Please click on the link below, print the completed complaint form, sign it, and send it with the required documentation to: Department of Business and Professional Regulation, 1940 North Monroe Street, Tallahassee, Florida 32399-0782. Pursuant to Section 455.225, Florida Statutes, a complaint is legally sufficient if it contains ultimate facts that show that a violation of this chapter, of any of the practice acts relating to the professions regulated by the Department, or of any rule adopted by the Department or a regulatory board in the Department has occurred. In most cases, if supporting documentation and signed complaint is not received within 30 days after submission, the complaint will be dismissed. Below, please find examples of needed supporting documentation for a selected number of the regulated professions.

Construction Contractors/Electrical Contractors: Copies of the contract, proof of payment (copies of the front and back of checks, invoices, receipts, etc.), inspections which document a code violation or major defect.

Professional Surveyors & Mappers: Signed and sealed copy of the survey in question, proof of the contractual agreement, proof of payment, etc.

Veterinarians: Completed and signed patient release form (last page of the printed complaint form).

Community Association Management: Contract between the manager and the association, documentary proof of any financial mismanagement allegations if made in the complaint, signed complaint - preferably from the association board. If the complainant is not a board member, it is recommended that they should address the allegations to their association board for review and appropriate action first.
