Cosmetologist Re-Examination (COSMO 3)

This application is used by applicants seeking an initial license who have previously failed any part of the Florida cosmetology examination.

Application Requirements:
 
  • EDUCATION: For cosmetology applicants that only completed 1,000 hours of education in Florida prior to taking the initial examination, you must return to school and complete the remaining 200 hours of education before you can apply for reexamination. The school official must sign the portion of the application verifying completion of 1,200 cosmetology education hours and the completion date.
  • EXAMINATION: If you have failed the examination, you are required to submit a reexamination application before you can schedule another examination with Pearson VUE. To reapply online or obtain a retake application please visit Cosmetology - Apply For A License. Applicants must complete both portions of the examination within a two-year period from the date of the first attempt of either portion of the exam. If both portions of the examination are not passed within a two year period, the applicant will be required to pass both portions of the examination again. If the applicant fails either portion of the examination on the first attempt, the applicant can submit a reexamination application without any additional signatures from the cosmetology school. Remedial hours may be required with verification by the school.   • HIV/AIDS COURSE: If the initial HIV/AIDS course completion certificate submitted with your original Florida cosmetology examination application is over two years old, you must complete another initial HIV/AIDS course, and submit the course completion certificate with your re-examination application. All initial HIV/AIDS courses must be four hours long. For more information, please refer to our list of board-approved Initial HIV/AIDS Courses.
  • FEE: Pay the required fee as provided in the application. If applying online, you can pay with a credit card or electronic check. If mailing your application, please include a check payable to the Department of Business and Professional Regulation. Military veterans, their spouses and Florida National Guard members may be eligible for a fee waiver, fee reimbursement, and/or fee discount.   • APPLICATION: Complete the application by clicking on the "Online Services" link or the "Printable Application" link at the bottom of the page.

    Note: First time DBPR Online Services users will need to create an account and then link their Exam Candidate ID#. Instructions and additional information can be found by selecting "DBPR Online Services Features and Steps" on the Main Menu. Returning users will need to make sure their Exam Candidate ID# is linked before proceeding.
