Barber HIV/AIDS Initial and Continuing Education Course Approval (BAR 7)
This application is used by individuals or companies that would like board approval to offer an HIV/AIDS education course for the purpose of initial licensure or continuing education to licensed barbers.

REQUIREMENTS:
A detailed course outline, course materials, and a
sample course completion certificate must be included. The sample course
completion certificate must include spaces for the student''s name, title of the
course, course approval number, course completion date, and total number of
course hours. All providers of HIV/AIDS courses for initial licensure or
continuing education for barbers must be approved prior to offering their
courses.
