Cosmetology Continuing Education Course (COSMO 9)
This application is used by individuals or companies that would like board approval to offer continuing education courses to licensed cosmetologists or registered specialists.

REQUIREMENTS: A sample course completion certificate must be included. The sample course completion certificate must include spaces for the licensee's name, license number, provider's name, title of the course, course approval number, course completion date, and total number of course hours. Instructor resume, a detailed course outline, a list of all reference and source materials, along with their publication date(s), a copy of an evaluation assessing the learning experience, and instructional methods and course materials. Applications must be received at least 60 days in advance of a board meeting for consideration by the board. All providers of continuing education courses must be approved by the Board of Cosmetology prior to offering continuing education courses. - FEE: No fee is required for cosmetology continuing education course applications.
