Cosmetology Initial HIV/AIDS Course (COSMO 8)
This application is used by individuals or companies that would like board approval to offer an HIV/AIDS education course for the purpose of initial licensure or registration.

REQUIREMENTS: A detailed course description course outline, course objectives, and a sample course completion certificate must be included. The sample course completion certificate must include spaces for the licensee's name, license number, provider's name, title of the course, course approval number, course completion date, and total number of course hours. At least one complete copy of the course in its final form, as it will presented to the licensee, if approved, shall be submitted with the course application. Applications must be received at least 60 days in advance of a board meeting for consideration by the board. All providers of the initial HIV/AIDS education course must be approved by the Board of Cosmetology prior to offering the initial HIV/AIDS education course. FEE: No fee is required for cosmetology initial HIV/AIDS education course applications. APPLICATION: Complete the application by
