Apply for a Seating Establishment Plan Review Only

To begin Florida's food service licensing process, the law requires the division to review facility plans for sanitation and safety concerns. Plan review is required when the establishment is newly licensed. The application selected is for a new seating Food Service Establishment plan review only and requires a separate application for license.

Application Requirements:
  • Application: choose the correct application for your situation (Guide to Food Service)
  • For Plan Review:

    • Plans: scale drawing or architectural plans showing and labeling all equipment, restrooms, and exits (see Plan Review Guide)
    • Menu: a copy of your menu, printed, typed, or handwritten

    Please provide ALL required items at time of application to avoid delay.

  • Note: This application does not include a license and provides plan review approval only. You can use this for a remodel on an existing establishment. For new establishments we recommend the License and Plan Review at the same time (COMBO) application, or you will need to follow-up with a license application once plan review is approved.

    The follow-up license application is only available through your online account once the plan review is approved.
