Limited-Service Veterinary Medical Practice Clinic Registration (VM 4)
This application is used by individuals or companies that are licensed to offer Limited Service clinics to register their clinic sites with the department. Each individual clinic must be registered with the department. The registration is valid through May 31 of each year, regardless of when the clinic was operational. You must submit this application to add and/or re-register additional clinics for limited service veterinary medical practice.

ADDITIONAL INFORMATION & REQUIREMENTS: Each limited service clinic must be registered with the department at least 28 days prior to the clinic. Clinics
held inside climate controlled buildings may be held up to 2 days per week for
up to 8 hours per day. All other clinic
locations are limited to once every two weeks and no more than four hours in
any one day for any single location. The registration is valid through May 31st of
each year regardless of when it was submitted or when the clinic was
operational. Read Rule 61G18-15.007, 61G18-15.0071, and 61G18-15.0072, Florida Administrative Code, which outlines the minimum standards for a limited service practice. - FEE: Pay the required fee as provided in the application. If applying online, you can pay with a credit card or electronic check. If mailing your application, please include a check payable to the Department of Business and Professional Regulation. If your business was closed due to damage from Hurricane Helene or Hurricane Milton, you may be eligible for a waiver of the fee for relocating your business; additional details are in the application.
