- FEE: Pay $10 per lien, per license.
- APPLICATION: Complete form DBPR ABT-6022 by clicking on "Printable Application" at the bottom of this page (detailed instructions included). If you already have a copy of the detailed instructions, you may prefer to obtain the form without the instructions in ABT-6022 PDF or ABT-6022 Word format.
- SECURITY AGREEMENT: Submit a copy of the Security Agreement, if applicable.
- PROMISSORY NOTE: Submit a copy of the promissory note outlining the terms of agreement, if applicable.
- ASSIGNMENT AGREEMENT: Submit a copy of the Assignment Agreement signed by the new secured party, if applicable.
- ASSUMPTION AGREEMENT: Submit a copy of the Assumption Agreement singed by the new debtor, if applicable.
- RENEWAL/EXTENSION: Submit a copy of the renewal or extension agreement, if applicable.
- SUBMIT APPLICATION: Submit your application to the Department of Business and Professional Regulation, 2601 Blair Stone Road, Tallahassee, Florida 32399-1021.
- LIEN SEARCH: Submit this form for your lien search, along with a $20 fee: ABT-6023 PDF format.
