CROSS TRAINING PROGRAM APPROVAL (BCAIB 5)
This application is used by approved providers who would like to have their cross-training program approved by the board.
Application Requirements: |

REQUIREMENTS: Instructor resumes and a detailed course syllabus must be
included. Also include a sample course
completion certificate and instructor resumes. Applications must be
received at least 60 days in advance of a board meeting for consideration by
the board. FEE: For training
program approval submit a fee in the amount of $25 per every 100 program hours,
no more than $100. Make check payable to the Department of Business and
Professional Regulation. If applying
online, you can pay with a credit card or electronic check. If mailing your
application, please include a check payable to the Department of Business and
Professional Regulation. APPLICATION: Complete the application by clicking on

link at the bottom of the page.
