Asbestos Business - License Maintenance/Status Change Form (ALU 4)

This application is used by business entities that wish to add or change a qualifying agent to an existing business, add or change a qualifying agent to a new business, change from a business to an individual, reactivate to qualify an existing business, reactivate to qualify a new business, reactivate an individual license, or change your current business name to a new business name.

  • FEE: Pay the required fee as provided in the application. If applying online, you can pay with a credit card or electronic check. If mailing your application, please include a check payable to the Department of Business and Professional Regulation.
