Complete this application if you wish to host a professional Boxing, Kickboxing, or Mixed Martial Arts event.

LICENSED PROMOTER:
An
Application for Permit may be submitted by a promoter licensed in Florida.
INSURANCE:
By weigh-in, promoter must provide written evidence of
medical/ surgical/hospitalization insurance and accidental death insurance for
all participants (minimum of $20,000 in benefits in each coverage category for
each participant). Promoter is
responsible for any deductible.
PERMIT FORM:
Attach a completed Application
for Permit form.
FEE:
Pay the $1,800 non-refundable fee by credit card or
electronic check.
APPLICATION:
