Permit for Live Event

Complete this application if you wish to host a professional Boxing, Kickboxing, or Mixed Martial Arts event.

Application Requirements:
  • LICENSED PROMOTER: An Application for Permit may be submitted by a promoter licensed in Florida.

  • INSURANCE: By weigh-in, promoter must provide written evidence of medical/ surgical/hospitalization insurance and accidental death insurance for all participants (minimum of $20,000 in benefits in each coverage category for each participant). Promoter is responsible for any deductible.

  • PERMIT FORM: Attach a completed Application for Permit form.

  • FEE: Pay the $1,800 non-refundable fee by credit card or electronic check.

  • APPLICATION: Complete the application by clicking on the "Online Services" link below.
