Employer Waiver

This waiver is for employer's use only and it is intended for employers seeking to change the break hours of regular workforce minors, and for employers who are seeking a gas waiver for minors who have completed an appropriate safety training course, to dispense supplemental oxygen.

Application Requirements:
 
  • Print Waiver Application or Apply for a Waiver Application below.
  • Complete the employer''s information, including the employer''s FEIN number.
  • Complete the form including necessary signatures.
  • Submit supporting documents: a letter with the proposed waiver request and a list of participating minors with birth dates.
  • Scan the documents to your computer.
  • Email application and supporting documents to childlaborwaivers@myfloridalicense.com.
