Participant License for an Individual without a Social Security Number

Complete this application if you wish to be licensed as a Boxing, Kickboxing or Mixed Martial Arts participant.

Application Requirements:
 
  • AGE: Applicant must be at least 18 years old.
  • MEDICAL EXAMINATION/TESTING REQUIRED TO PARTICIPATE:
    Laboratory results (negative result acceptable up to 1 year):

    • Hepatitis B surface antigen
    • Hepatitis C antibody
    • Rapid Human Immunodeficiency Virus/AIDS (HIV)
  • Dialated eye examination (acceptable up to 1 year):

    Age 40 and older:

    • Normal EKG clearing applicant to compete (acceptable up to 1 year)
    • Normal MRI of the brain

    Pre-match physical examination:

    • Participant will be examined by Commission physician at weigh-in
    • Female participants - pregnancy test will be conducted at weigh-in
  • FEE: Pay $25 non-refundable fee by credit card or electronic check.
