Apply for a Mobile Food Dispensing Vehicle (MFDV) Plan Review Only

To begin Florida's food service licensing process, the law requires the division to review facility plans for sanitation and safety concerns. Plan review is required when the establishment is: 1) newly built, 2) converted from another use, 3) remodeled or 4) re-opened after being closed at least 1 year. The application process described below is for a new Mobile Food Dispensing Vehicle plan review only. **This application requires a follow-up license application. For the vast majority of applicants the New MFDV License and Plan Review at the Same Time choice is the best way to become licensed.

Application Requirements:
 
  • Application: You are on track to apply for a Mobile Food Dispening plan review (without a license). Please take a moment to briefly review our Guide to MDFVs page. It covers the application process, plan review requirements and includes tips to ensure your application is approved quickly. When ready, apply using the links at the bottom of this page.

  • Plan Review: To complete this application you will need to upload digital copies of your plans (drawing) and menu during the online application process.
    Scaled Drawing: A scaled drawing of the vehicle showing the all of the food service related equipment/fixtures. A side view of the vehicle is also required.
    Menu: A copy of the proposed menu.

  • Commissary: A commissary is a licensed commercial food establishment which provides required services to the mobile food dispensing vehicle. If applicable to your operation The commissary form is available here: Commissary Notification Form

  • Note: This application does not include a license and provides plan review approval only. For new establishments we recommend the License and Plan Review at the same time or follow-up with a license application once approved.page.
    The follow-up license application is only available through your online account once the plan review is approved.
