Term Renewal of Federal ID Card - Boxing - Individual with Social Security Number

RENEWAL APPLICATION:
ABC Federal ID was previously issued to applicant.
AGE:
Applicant must be at least 18 years old.
RESIDENCY:
Apply with a Commission based on your residency.
Apply with the Florida Athletic Commission

Attach scanned copy of government-issued photo identification.

driver license photo or a passport photo.)
FEE:
