Apply for a MFDV License with Plan Review
Florida law requires owners of new public food service and new owners of existing establishments to obtain a license from the division before operating. The application below is for a Mobile Food Dispensing Vehicle license with plan review

Mobile Food Dispensing Vehicle Application: You are on track to apply for a new Mobile Food Dispensing Vehicle (MFDV) License with Plan Review. If you have not already, please briefly review our Guide to Mobile Food Dispensing Vehicles page. It covers the licensing process, application requirements and includes tips to ensure your application is approved quickly.
- Plan Review: To complete the plan review portion of this application you will need to upload digital copies of your plans and menu during the online application process.
- Scaled Drawing: A scaled drawing of the vehicle showing all of the food service related equipment/fixtures. A side view of the vehicle is also required.
- Menu: A sample menu.
- Commissary: A commissary is a licensed food service which provides required services to the mobile food dispensing vehicle. If applicable to your operation complete the Commissary Notification Form.
- Applying Online: For fastest turnaround apply online using the "Apply Online!" link at the bottom of the page.
