Registered Sheet Metal Contractor - Reinstate Null and Void Registration (CILB 26)

This form is used by individuals who are requesting the Department of Business and Professional Regulation to reinstate their null and void license. Applicants must demonstrate to the Department that they failed to timely renew their license due to an economic hardship or illness. If the Department denies your application, you will receive notification from the Department outlining your hearing rights. If your application denial is upheld, you will be required to apply for a new license, which may include retaking an examination.

Application Requirements:

  • LOCAL LICENSE: For registered contractors only, please submit copy of current local occupational license and current local competency card.
  • DEFINITIONS: A license becomes "null and void" after a licensee fails to renew the license for two consecutive licensure cycles. A null and void license cannot be reinstated unless the applicant demonstrates to the Department that he or she failed to renew the license due to an illness or economic hardship. There is no specific definition for "illness" or "economic hardship." The Department reviews each application on a case-by-case basis.
  • FEE: Pay the required fee as provided in the application. If applying online, you can pay with a credit card or electronic check. If mailing your application, please include a check payable to the Department of Business and Professional Regulation.

  • APPLICATION: Complete the application by clicking on the "Online Services" link or the "Printable Application" link at the bottom of the page.

Note: First time DBPR Online Services users will need to create an account and then link existing license(s). Instructions and additional information can be found by selecting "DBPR Online Services Features and Steps" on the Main Menu. Returning users will need to make sure their license is linked before proceeding.
