Slot Machine Professional Employee Occupational License

Florida Slot Machine Professional Employee Occupational licenses shall have an effective date of July 1st and shall be valid for a period of three state fiscal years.

Application Requirements:
 
  • APPLICATION: PMW-3410 Individual Occupational License Application form.

  • FEE: The license fee is $100 for a three-year license.

  • IDENTIFICATION: Two forms of identification as required by Rule 61D-14.010, Florida Administrative Code.

  • FINGERPRINTS: All new applicants must submit fingerprints electronically or using a physical fingerprint card. If submitting physical fingerprint cards, completed fingerprint cards (FD-258) must be mailed to the Division with your application and the $37.25 fingerprint processing fee.

    ORI FL925185Z must be used when submitting electronic fingerprints for Slot Machine Occupational Licenses.
