Slot Machine Educational Facility License - Renewal

For educational facilities that conduct specific training courses on slot machines.

Application Requirements:
 
  • APPLICATION: A DBPR PMW-3422 Slot Machine Certified Educational Facility License Application form must be completed. Complete the application by clicking on the "Printable Application" or "Online Services" link at the bottom of the page.
  • LICENSE TERM: Florida Slot Machine Educational Facility Licenses are valid from May 1st through June 30th of the following year.
  • FEE: For educational facilities that conduct specific training courses on slot machines. Application fee is $100 for a one-year license, $150 for a three-year license.
  • AUTHORIZATION FOR RELEASE OF INFORMATION: must be completed PMW-3460.
  • NOTICE FOR APPLICANTS SUBMITTING FINGERPRINTS WHERE CRIMINAL RECORD RESULTS WILL BE USED FOR LICENSING PURPOSES
  • A COPY OF CERTIFICATION FROM THE DEPARTMENT OF EDUCATION MUST BE INCLUDED.
