Talent Agency - Location Change (TA 3)

This form is used to notify the department when a talent agency wishes to change its name or address. Either change requires department approval.

Application Requirements:
  • SURETY BOND: Submit an updated surety bond (Bond must show new agency name, address, phone number, etc.).

  • LICENSE: Return your original talent agency license with the application.
  • FEE: Pay $25. Make check payable to the Department of Business and Professional Regulation.

  • APPLICATION: Complete the application by clicking on the "Printable Application" link at the bottom of the page.
