Request for Inactive Status or Waiver of Active Operation Requirements for Quota Alcoholic Beverage Licenses

Used to request placement of a quota alcoholic beverage license in an inactive status, request a one-time inactive waiver or extension of active operation requirements, or to request a waiver or extension of active operation requirements under specific conditions as required by 561.29, Florida Statues.

  • FEE: Pay $35 fee.
  • APPLICATION:  Complete form DBPR ABT-6027 by clicking on "Printable Application" at the bottom of this page (detailed instructions included). 
  • SUBMIT LETTER:  Submit your application to one of our district offices by mail, hand delivery or contact the local office to make an appointment.
