Broker (Sole Proprietorship Only) - Business Address Change (RE 12)

This transaction is used to notify the department of changes to the business address of a sole proprietorship broker.

Application Requirements:
  • APPLICATION: The request may be completed through your "Online Service" account or by completing a "Printable Application". Please choose one of the options at the bottom of the page.
  • PLEASE NOTE: The printable application has two address change options to choose from:

    One option is for address changes without requesting issuance of an updated license and

    the other is for address changes with issuance of an updated license.

    Please be sure to select the option most appropriate for your needs.
