Appraisal Management Company-Amendment Form (FREAB 3)
The Appraisal Management Company Amendment form is to be used when an existing AMC has a new authorized representative to add to their records or has to remove an authorized representative from their records.

- Application: The application is only available in paper format at this time. Please go to the bottom of the requirement page and click on Printable Application. Please use one application per Authorized Representative.
FINGERPRINTS:
An applicant must have a background check as part of
the licensing process. You must submit your
fingerprints using a Livescan Service Provider registered with
FDLE immediately after submitting your
application for licensure. It may take up to five days for your fingerprint results to be
received by the Department after they are submitted to the Florida Department
of Law enforcement (FDLE). Please note
that the Florida Department of Law Enforcement requires that an application for
licensure should precede the submission of your fingerprints. To learn more

- Background Questions: The background questions are to be completed by new authorized representatives: If you answer Yes to any of the four background questions, please complete the detail section of the form and submit the appropriate supporting documents to prevent delay in the processing of the application. If you intend to answer No due to the charge being dismissed, nolle prosse, no action, not guilty, sealed, or expunged, please be prepared to provide proof due to the fingerprint report does not always disclose the final disposition or outcome of an arrest. The background information is reviewed by the department and in some cases may require the Florida Real Estate Appraisal Board approval.
