Controlling Person License By Endorsement (ELC 1)
This application is used to apply for licensure as a controlling person by endorsement with an active license in another state or territory. A controlling person is a person who has the power to manage a company, with direct or indirect control of 50% or more of the voting securities, power to endorse any negotiable payment on behalf of the company or is authorized to enter into a contractual relationship with a client company.

NOTE:
A controlling person''s license shall exist only in
conjunction with a licensed Employee Leasing Company. If the applicant is
applying to be the Controlling Person of a new company, the company application
must be submitted at the same time.
AGE:
Must be at least 18 years of age.
EDUCATION/EXPERIENCE:
Applications for an employee Leasing Controlling Person
by endorsement are required to have an active certification of licensure issued
by another state or territory of the United States and provide documentation
that show:
1. That the
applicant has been actively licensed for at least 5 years. 2. The
applicant''s license was active within 2 years of applying for licensure in this
state; and
3. There are
no pending disciplinary action pending and all sanctions of any prior
disciplinary actions have been satisfied.
FINANCIAL/CREDIT:
Complete an IRS Form 8821 for each controlling person
applicant. Submit an individual credit report from a nationally recognized
credit reporting agency, which includes a public records statement that records
have been checked at local, state, and federal levels.
- FINGERPRINTS: An applicant must have a background check as part of the licensing process. You must submit your fingerprints using a Livescan Service Provider registered with FDLE immediately after submitting your application for licensure. It may take up to five days for your fingerprint results to be received by the Department after they are submitted to the Florida Department of Law enforcement (FDLE). Please note that the Florida Department of Law Enforcement requires that an application for licensure should precede the submission of your fingerprints. FEE:
Pay the required fee as provided in the application. Please
include a check payable to the Department of Business and Professional
Regulation. Military veterans, their spouses and Florida National Guard members
may be eligible for a fee waiver, fee reimbursement, and/or fee discount.
