SLOT MACHINE PROFESSIONAL EMPLOYEE OCCUPATIONAL LICENSE - TEMPORARY

Occupations include (but are not limited to): slot operations managers; slot shift managers; floor supervisors; slot tech managers; slot tech supervisors; slot technicians; slot attendants; facility (slot machine licensee) management; slot machine officers, directors and shareholders; chiefs of security; security officers; surveillance directors, investigators, supervisors, operators, and technicians; maintenance supervisors; controllers; count team employees; cage cashiers; count room managers and supervisors; cage managers and supervisors; information systems managers; systems analyst supervisors; operations analyst supervisors; revenue audit managers and supervisors.

Application Requirements:
 
  • APPLICATION: A DBPR PMW-3410 Individual Slot Machine Occupational License Application form must be completed. Complete the application by clicking on the "Printable Application" link at the bottom of the page. Please hand-deliver or mail your application to the pari-mutuel facility where you will be working. To locate the address of the slot machine facility closest to you please review the facility list provided.

  • LICENSE TERM: Florida Slot Machine Licenses are valid from May 1st through June 30th of the following year.
  • FEE: Application fee is $50 for a one-year license, $100 for a three-year license, plus a $40.50 fingerprint processing fee. Application fees and fingerprint fees must be submitted to the slot machine licensee (permitholder). An honorably discharged disabled veteran from one of the time periods specified in s.1.01(14), F.S., or the unremarried spouse of a deceased disabled veteran meeting this criteria, may be exempt from license fees up to $50.00. Fingerprint fees are not exempt.

  • IDENTIFICATION: Two forms of identification as required by Rule 14.010, Florida Administrative Code 61D-14.010.
  • AUTHORIZATION FOR RELEASE OF INFORMATION: must be completed: PMW-3460.
  • AGE REQUIREMENT: Applicants for Slot Machine licensing must be 21 years of age or older.
  • FINGERPRINTS: Slot Machine General & Professional Applicants

    must submit his/her fingerprints for a criminal history background check and pay the $40.50 fingerprint processing fee when submitting an initial application. Every three (3) years after the initial fingerprint submission, upon renewal, the applicant must pay a $16.50 for his/her to be resubmitted to the Florida Dept. of Law Enforcement for an updated criminal history background check.

    You may submit your fingerprints electronically through a law enforcement agency that has a live scan or hard card scanning device. If you are able to submit your fingerprints electronically, please submit them with the following required ORI number: FL923230Z.

    Fingerprints will be electronically retained for future submissions to the FDLE. Fingerprints can be mailed to DBPR; Division of Pari-Mutuel Wagering; Licensing Section, 1940 North Monroe Street, Tallahassee, FL 32399-1037.
