PARI-MUTUEL/CARDROOM EMPLOYEE COMBO LICENSE - TEMPORARY

All food service, security, maintenance, and mutuel teller employees who require access to restricted areas of the track as well as the cardroom.

Application Requirements:
 
  • APPLICATION: Individuals applying for the first time must complete a DBPR PMW-3120 Individual Occupational License Application form. Complete the application by clicking on the "Printable Application" or "Online Services" link at the bottom of the page. You may also hand-deliver or mail your application to the pari-mutuel facility where you will be working. To locate the address of the slot machine facility closest to you please review the facility list provided.

  • LICENSE TERM: Florida Cardroom Occupation Licenses are valid from May 1st through June 30th of the third year.
  • FEE: License fee $15. For a three-year license. (make check payable to DBPR). An honorably discharged disabled veteran from one of the time periods specified in s.1.01(14), F.S., or the unremarried spouse of a deceased disabled veteran meeting this criteria, may be exempt from license fees up to $50.00. Fingerprint fees are not exempt.
  • FINGERPRINTS: All applicants must submit his/her fingerprints for a criminal history background check and pay the $40.50 fingerprint processing fee when submitting an initial application. Every five (5) years after the initial fingerprint submission, upon renewal, the applicant must pay a $16.50 for his/her to be resubmitted to the Florida Dept. of Law Enforcement for an updated criminal history background check.

    You may submit your fingerprints electronically through a law enforcement agency that has a live scan or hard card scanning device. If you are able to submit your fingerprints electronically, please submit them with the following required ORI number: FL920630Z.

    Fingerprints will be electronically retained for future submissions to the FDLE. Fingerprints can be mailed to DBPR, Division of Pari-Mutuel Wagering; Licensing Section, 1940 North Monroe Street, Tallahassee, FL 32399-1037.
