General Individual Occupational License - Initial

Positions include, but are not limited to: athletic trainer, ball boy, ball maker, blacksmith, plater, cesta maker, exercise person, groom, head lead out/lead out, horse clipper, hot walker, kennel helper, maintenance employee (with access to restricted areas), outrider, pony rider, security employee, stable agent, stable help, vendor representative (with access to restricted areas), veterinarian assistant, mutuels clerk/teller, totalisator employee, moneyroom employee and any other professional person with access to restricted areas.

Application Requirements:
 
  • APPLICATION: Individuals applying for the first time must complete a DBPR PMW-3120 Individual Occupational License Application form. Complete the application by clicking on the "Printable Application" or "Online Services" link at the bottom of the page. You may also hand-deliver or mail your application to the pari-mutuel facility where you will be working. To locate the address of the slot machine facility closest to you please review the facility list provided.
  • PROFESSIONALS INCLUDING: doctor, nurse, paramedic, emergency medial technician (EMT), physician's assistants, and veterinarian (including track veterinarian) must have a Florida professional license with the proper state agency, prior to occupational licensure by the Division of Pari-Mutuel Wagering.
  • LICENSE TERM: Florida Pari-Mutuel Licenses are valid from May 1st through June 30th of the third year.
  • FEE: General Individual Occupational License $15. For a three-year license. (make check payable to DBPR). An honorably discharged disabled veteran from one of the time periods specified in s.1.01(14), F.S., or the unremarried spouse of a deceased disabled veteran meeting this criteria, may be exempt from license fees up to $50.00. Fingerprint fees are not exempt.

  • FEE:

    FINGERPRINTS: Fingerprints are required upon initial licensure and every five years thereafter. A fee of $40.50 is due with each fingerprint card submission. A fingerprint card may be obtained at any Pari-Mutuel facility; your local law enforcement office (prints must be taken on an "Applicant" fingerprint card); by contacting the division at the number listed below; or you may submit your fingerprints electronically through a law enforcement agency that has a live scan or hard card scanning device. If you are able to submit your fingerprints electronically, please submit them with the following required ORI number: FL920630Z.

    Fingerprints will be electronically retained for future submissions to the FDLE. Fingerprints can be mailed to DBPR; Division of Pari-Mutuel Wagering; Licensing Section, 1940 North Monroe Street, Tallahassee, FL 32399-1037.

    Instead of a fingerprint card; applicants 70 years of age and over may submit $24.00 for a criminal history check.

  • NOTE: Certified law enforcement or corrections officers working in security or safety positions do not require an occupational license, except when working as the Chief of Security.
