Slot Machine/Pari-Mutuel/Cardroom Combo Professional Individual - Initial

For anyone who works in a professional-level position and needs access to the slot machine area and other restricted areas of the pari-mutuel facility to perform his/her duties. For example, a slot machine technician who is also a card dealer should apply for a slots combo professional license Occupations include (but are not limited to): all professional-level slots, pari-mutuel, and cardroom license occupations.

Application Requirements:
 
  • APPLICATION: A DBPR PMW-3410 Individual Slot Machine Occupational License from must be completed. Complete the application by clicking on the "Printable Application" or "Online Services" link at the bottom of the page.

  • LICENSE TERM: The division shall issue a slot machine occupational license for a period of one year or three years. Applications shall be accompanied by the corresponding license fee, beginning on July 1 of each year and expiring on June 30 of the anniversary year for the license.

  • AGE REQUIREMENT: Applicants for Slot Machine licensing must be 21 years of age or older.

  • FEE: The license fee is $50 for a one-year license or $100 for a three-year license.
  • IDENTIFICATION: Two forms of identification as required by Rule 14.010, Florida Administrative Code 61D-14.010.

  • FINGERPRINTS: Individual occupational license applicants must submit fingerprint cards and pay the $37.25 fingerprint fee the first year. Fingerprints will be electronically retained for future submissions to the FDLE. Every 3 years thereafter, upon renewal, the applicant must pay a $13.25 resubmission fee for his/her fingerprints to be resubmitted to the Florida Dept. of Law Enforcement for an updated criminal history background check.

    If not eligible for fingerprint resubmission, physical fingerprint cards can be mailed to DBPR, Division of Pari-Mutuel Wagering; Licensing Section, 2601 Blair Stone Road, Tallahassee, FL 32399-1037.

  • You may alternatively submit your fingerprints electronically through a law enforcement agency or an FDLE approved Livescan Device Vendor. If you are able to submit your fingerprints electronically, please submit them with the following required ORI number: FL923230Z. Livescan Device Vendors List.

  • NOTICE FOR APPLICANTS SUBMITTING FINGERPRINTS WHERE CRIMINAL RECORD RESULTS WILL BE USED FOR LICENSING PURPOSES

  • Fees may be paid by check or money order only and should be made payable to DBPR in U.S. funds. Application packages may be submitted at a PMW field office or mailed to the following address:
