Event Permit - Live Event

Complete this application if you are a licensed Promoter and you wish to host a Boxing, Kickboxing or Mixed Martial Arts event.

Application Requirements:
  • INSURANCE: Insurance requirements: The promoter is required to obtain medical insurance and accidental death insurance for all participants. The policy must cover each fighter for a minimum of $20,000 in benefits for each category.

  • FEE: Pay the $1,800 non refundable fee (check or money order payable to Florida State Boxing Commission).
  • APPLICATION: Complete the application by clicking on the "Printable Application" link at the bottom of the page.
